The 17th International Conference on Integrated Care “Building a platform for integrated care: delivering change that matters to people” takes place in Dublin, Ireland 08 to 10 May 2017, and marks the end of the SIMPATHY project. At the same time, this conference can be seen as the start of the movement for change initiated by SIMPATHY. At the conference, we aim to launch the first Special Interest Group (SIG) on polypharmacy management and an interactive workshop will be designated to the steps that need to be taken after the SIMPATHY project.
Additionally, FCRB –Clinic Biomedical Research Foundation- and University of Peloponnese, two partners of the SIMPATHY consortium, will have submissions presented in the main conference programme / poster sessions.
